STEPS TO SUBMIT AN ONLINE APPLICATION
PLEASE READ INSTRUCTIONS CAREFULLY
OTHERWISE, APPLICATION WILL BE BLANK
WHEN IT ARRIVES IN THE CLINIC EMAIL.

  1. Download the Application in PDF Format to your computer. Save it to your computer (i.e. save it in the name of your pet).Fill out the form and SAVE IT AGAIN!! Close the application. Re-open the application, if all your information is on it, you have saved it correctly and NOW you can email it back to the clinic.
  2. You may also fax your Completed Application to 704-868-0010. You must have the most recent Adobe Reader. To download, please visit http://get.adobe.com/reader/.
  3. Once you have submitted the form, please proceed to our Pay Now page.
  4. You MUST call the clinic for a surgery date once the application and payment are submitted! 
  5. If you are submitting an application through a BLITZ, you will not be able to pay online as the Blitz prices are not posted in PayPal.  Once you have spoken to the tech at the clinic and submitted payment, a surgery date will be scheduled.

EFFECTIVE IMMEDIATELY:
THERE WILL BE A $20.00 FEE CHARGED FOR ALL RESCHEDULED APPOINTMENTS.
